Contestants (18 and over): 

• 1) Photo ID
• 2) Completed Personal Release (6 pages)
• 3) Completed Animal Questionnaire and immunization papers (if applicable)

Contestants (UNDER 18 MINORS):

• 1) Photo ID (For minors: may include school ID or yearbook photo)
• 2) Completed  Personal Release (6 pages) with parent signature
• 3) If the minor contestant’s parent or guardian will not be at the audition:

  -  Notarized and completed  Guardianship and Medical Authorization for Minors form

         AND

  -  Make sure page 6 of  Personal Release Form (6 pages) is signed by parent AND third-party

Family and Friends (18 and over):

• 1) Photo ID
• 2) Completed  Family and Friends Release (3 pages)

Family and Friends (UNDER 18 MINORS):

• 1) Photo ID (For minors: may include school ID or yearbook photo)
• 2) Completed Family and Friends Release (3 pages) with parent signature
• 3) If the minor’s parent or guardian will not be at the audition:

  -  Notarized and completed  Guardianship and Medical Authorization for Minors form

         AND

  -  Make sure page 3 of Family and Friends Release (3 pages) is signed by parent AND third-party
