I am auditioning alone (solo act) or with three people or less.

You must bring a photo ID to the audition and fill out the Personal Release.

To save time on your audition day, the best way to fill out the release for a solo act or an act of three or less is electronically.

If you are over 18, you can sign into your account here by using your email and password and following the link to the Electronic Personal Release Form.  Everyone in your act needs to be together to electronically sign the form at the same time.

If you are under 18 or this method doesn’t work for you for any reason, you can just download and fill out the release form.  The form is a fill-able PDF so you fill it out on your computer and print it or you can print it and fill it in by hand and bring it with you to the audition. Each member needs to fill out their own release.

If you or anyone in your group is under 18, they will need to fill out this additional paperwork.
