On this page, you’ll find all the paperwork and forms you will need to audition for America’s Got Talent.

Everyone coming to the audition, whether auditioning or not, will need to fill out the proper paperwork and have a photo ID. It’s best to bring a copy of the ticket that we email to you after you compete your registration for a city.

If you are under 18 years old you will need to bring additional paperwork.

If you are eligible, your best bet is to electronically sign the paperwork from your desktop computer at home.  Then you just need to bring a photo ID and your ticket to the audition.  If you are not eligible, try to download the paperwork and fill it out at home. If you are having problems printing the paperwork, don’t worry we will have copies at the the auditions. But having it prepared and signed before you come will save you a lot of time on the day of your audition.

TO COMPLETE THE CORRECT PAPERWORK, PICK FROM THE FOLLOWING:

I am auditioning alone (solo act) or with less than 5 adults and/or 5 minors.

You must bring a photo ID to the audition and fill out the Personal Release.

To save time on your audition day, the best way to fill out the release for a solo act or a group consisting of no more than 5 adults and/or 5 minors is to sign electronically. It’s best to do this on a desktop computer because it does not work with all mobile devices. By signing the paperwork electronically before you come, you will be expedited on the audition day.

You can sign into your account here by using your email and password and following the link to the Electronic Personal Release Form.  Groups should register under a single person’s sign in and they can share that log in with all the members to sign the Electronic Paperwork.

If you have more than 5 adults or 5 minors in your group or this method doesn’t work for you for any reason, you can simply download and fill out the release form.  The form is a fill-able PDF so you fill it out on your computer and print it or you can print it and fill it in by hand and bring it with you to the audition. Each member needs to fill out their own release.

If you or anyone in your group is under 18, they will need to fill out this additional paperwork.

I am auditioning with an act of more than 5 adults and/or 5 minors

  • Only one member of the group should register online.  That person will be registering for the whole group and they will be the main contact for audition information from us.
  • Every member of the group will need to show a photo ID when they arrive.Tampa Auditions
  • You and everyone in your group need to fill out the Personal Release and bring it to the audition.
  • The form is a fill-able PDF so you fill it out on your computer and print it or you can print it and fill it in by hand

I am auditioning with more than 15 people in the same act.

Only one member of the group should register online.  That person will be registering for the whole group and they will be the main contact for audition information from us.

If you plan on coming to the auditions with more than 15 people in your act, we can help make sure you have all the paperwork you need to make your audition day go a lot smoother.  Please fill out the Large Group Form and we will be in touch before your audition day to help you prepare.

I have a friend or family member coming to support me.

supportersYou may bring friends and family members to support you. Just keep in mind they will not be allowed in the actual audition room (unless you are the parent or guardian of a minor who is auditioning).

  • All supporters will need to show a photo ID.
  • Supporters need to download the Family and Friends Release, sign the bottom, and bring it to the audition.
  • If you or anyone in your group is under 18, they will need to fill out this additional paperwork.

IMPORTANT INFORMATION ABOUT MINORS


FOR ANYONE UNDER 18 COMING TO THE AUDITION

1) All minors, contestants and supporters, must have permission from their parents or legal guardian to attend the auditions and should be accompanied by one of their parents or their legal guardian.

2) Both the minor and a parent or legal guardian must complete the Personal Release by downloading the form, or logging into their Contestant Profile and e-signing it (which will save time on your audition day).

3) Everyone under 18 attending the audition will be required to present a photo ID.

  • Minor contestants and supporters may use a school ID or acceptable substitute (such as a passport, yearbook page that includes a printed name with a photo AND a birth certificate).
  • If the minor is too young to have a photo ID, then the parent or guardian should present a photo ID & the child’s birth certificate that includes the parents name with the child’s name on the certificate.

If you are under 18 and our planning on attending the auditions without your parent or legal guardian (either as a performer or a supporter) there are very strict guidelines you must read carefully or your group will not be allowed to auditioned!

WHAT IF I AM UNDER 18 AND MY PARENT OR GUARDIAN CANNOT BE THERE?

FOR MINORS WHO ARE AUDITIONING WITHOUT THEIR PARENT OR GUARDIAN PRESENT

If the parent or legal guardian of the under 18 minor is unable to attend, and is allowing the minor to attend the audition under the supervision of a third-party (such as a coach) who is at least 21 years old, you MUST do two things:

1) Download and complete the Guardianship and Medical Authorization for Minors Form and get it NOTARIZED by a notary public (typically at banks or mailing stores, or search your local directory) and bring it with you on the day of your audition.

2) Designate the third-party acting as guardian on page 8/9 of the Personal Release.  Must be signed by the parent or legal guardian AND the third-party acting as guardian.

FOR MINORS WHO ARE COMING WITHOUT THEIR PARENT TO SUPPORT A FRIEND

This is discouraged, but if a parent or legal guardian of the under 18 minor is unable to attend, and is allowing the minor to attend the audition under the supervision of a third-party (such as a coach), you MUST do two things:

1) Download and complete the Guardianship and Medical Authorization for Minors Form and get it NOTARIZED by a notary public (typically at banks or mailing stores, or search your local directory) and bring it with you on the day of your audition.

2) Designate the third-party acting as guardian on page 3 of the Family and Friends Release.  Must be signed by the parent or legal guardian AND the third-party acting as guardian.

 

Animals

I am planning on bringing an animal to the auditions

1) You will need a copy of your inoculation records from your veterinarian verifying that your animal(s) have all of their current vaccinations.
