On this page, you’ll find all the paperwork and forms you will need to audition for America’s Got Talent.
Everyone coming to the audition, whether auditioning or not, will need to fill out the proper paperwork and have a photo ID.
NOTE: IF YOU ARE UNDER 18 – YOU NEED TO BRING ADDITIONAL PAPERWORK.
TO DOWNLOAD THE CORRECT PAPERWORK, PICK FROM THE FOLLOWING:
I am auditioning or I am part of a group auditioning.

- Every member of the group will need to show a photo ID.
- You and everyone in your group need to fill out the Personal Release and bring it to the audition. The form is a fill-able PDF so you fill it out on your computer and print it or you can print it and fill it in by hand.
- If you or anyone in your group is under 18, they will need to fill out this additional paperwork.
I am not auditioning, but I am a friend or family member coming to support an act.
You may bring friends and family members to support you. Just keep in mind they will not be allowed in the actual audition room (unless it is a minor who is auditioning).
- All supporters will need to show a photo ID.
- Supporters need to download the Family and Friends Release, sign the bottom, and bring it to the audition.
- If you or anyone in your group is under 18, they will need to fill out this additional paperwork.
I am planning on bringing an animal to the auditions
If your act includes any type of animal:

1) You will need a copy of your inoculation records from your veterinarian verifying that your animal(s) have all of their current vaccinations.

- Everyone under 18 attending the audition will be required to present a photo ID.
- Minor contestants and supporters may use a school ID or acceptable substitute (such as a yearbook page that includes a printed name with a photo AND a birth certificate). If the minor is too young to have a photo ID, then the parent or guardian should present a photo ID & the child’s birth certificate that includes the parents name with the child’s name on the certificate.
All minors, contestants and supporters, MUST have permission from their parents or legal guardian to attend the auditions AND must be accompanied by one of their parents or their legal guardian.
- FOR MINORS WHO ARE AUDITIONING:
- If the parent or legal guardian of the under 18 minor is unable to attend, and is allowing the minor to attend the audition under the supervision of a third-party (such as a coach), you MUST do two things:
- 1) Download and complete the Guardianship and Medical Authorization for Minors form and get it NOTARIZED by a notary public (typically at banks or mailing stores, or search your local directory) and bring it with you on the day of your audition.
- 2) Designate the third-party acting as guardian on page 6 of the Personal Release. Must be signed by the parent or legal guardian AND the third-party acting as guardian.
- If the parent or legal guardian of the under 18 minor is unable to attend, and is allowing the minor to attend the audition under the supervision of a third-party (such as a coach), you MUST do two things:
